Sheridan Beach Community Club WAIVER and RELEASE
ACKNOWLEDGE, agree, and represent that I understand the nature of the Activity and that my participant is qualified, in good health, and in proper
physical condition to participate in such Activity. I further agree and warrant that if at any time I believe conditions to be unsafe, I will
immediately discontinue further participation in the Activity.
2. FULLY UNDERSTAND that: (a) CLASS PARTICIPATION, ESPECIALLY ACTIVITIES INVOLVING SKILL PRACTICE AND TESTING (ESPECIALLY IN -
WATER PRACTICE AND TESTING), HAVE RISKS OF INJURY INCLUDING SERIOUS BODILY INJURY OR DEATH (Risks); (b) these Risks may be caused by my own actions or inactions, the actions or inactions of others
participating in the Activity, the condition in which the Activity takes place or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c)
there may be OTHER RISKS AND SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY
ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation or
that of the minor in the Activity.
3. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE
Sheridan Beach Community Club and their instructors/coaches/leaders conducting the Activity as well as their agents, employees,
third party contact instructors, or assistants; the certifying agency; other course
participants; and, the owners and lessors of premises on which the Activity takes place (each considered one of the
"RELEASEES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED
IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS, NEGLIGENT
SECURITY, AND RECREATIONAL OPERATIONS AND ACTIVITIES; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF
LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the Releases, I WILL
INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost
which any may incur as the result of such claim.

COVID-19 is an extremely contagious virus that spreads easily through airborne means and person to person contact. Federal health authorities and the State of Washington recommend social distancing to prevent the spread of COVID-19. Contraction of COVID-19 can lead to severe illness, personal injury, permanent disability, or death. Use of the Sheridan Beach Community Club facilities, as with any activity outside the home, could increase the risk of you or your child contracting COVID-19.

Our pool management and their staff make every effort to keep our facility clean and disinfected. However, as with any public facility, we cannot guarantee that any guest will be 100% protected from airborne illnesses such as COVID-19, colds, and flu, while using the facility.

Actions members should take to ensure their protection include:
• Social distancing (stay 6 feet from members not in your household)
• Mask-wearing when possible, except in pool
• Do not congregate in the hallway or in bathrooms
• Frequent hand washing and use of sanitizer
• Do not share toys or other items with members not in your household
• Stay home if you have or have had a fever, or have been exposed to anyone that has had symptoms or tested positive for COVID-19
• Disinfect common use items before and after use

By agreeing to this waiver, you voluntarily agree on behalf of yourself and all members of your household to assume the forgoing risks, and accept full responsibility for the actions of you and your household members. You agree to release Sheridan Beach Community Club, its contractors, Board, and employees from liability, and hold harmless same from any claims, actions, damages, costs, or expenses of any kind related to COVID-19 that could be perceived to have been derived from use of the facilities.
